Biosketch:
Dr. Paul M. Boylan obtained his Doctor of Pharmacy degree from the Wilkes University Nesbitt College of Pharmacy in May 2014. After graduation, he practiced as a community pharmacist for CVS Pharmacy in Southeastern Pennsylvania for one year. Dr. Boylan then completed a Pharmacy Practice Residency at Reading Hospital in West Reading, Pennsylvania. He is presently the PGY-2 Transitions of Care Resident at Nova Southeastern University and an Adjunct Professor at the College of Pharmacy. Dr. Boylan is privileged to provide transitional care services at his practice sites, Memorial Regional Hospital in Hollywood and Accountable Care Options in Boynton Beach. His care is centered on improving discharge medication reconciliation, researching the pharmacoeconomic outcomes of transitional care, and providing comprehensive patient education. Dr. Boylan’s current professional research focuses on the role of the pharmacist in preventing high risk readmissions in a tertiary community hospital.
